			<content:encoded><![CDATA[<div id="_mcePaste">Naturally people with epilepsy or the parents of children with epilepsy wish to know what the future holds. What are the chances of the fits abating? How long will it be necessary to take medication? There are a number of general points that should be made:</div>
<div id="_mcePaste">People with absences or grand mal seizures show the best response to anticonvulsant medication, while those with temporal lobe seizures do not respond as well.</div>
<div id="_mcePaste">Features associated with epilepsy which has a poorer outlook for &#8216;cure&#8217; are:</div>
<div id="_mcePaste">the more difficult the seizures are to control in the first place;</div>
<div id="_mcePaste">the longer the person has seizures;</div>
<div id="_mcePaste">the presence of more than one seizure type;</div>
<div id="_mcePaste">the association with mental retardation, readily identifiable abnormalities of the nervous system and psychological problems.</div>
<div id="_mcePaste">As a general rule, the outlook for people with primary (idiopathic) epilepsy is better than that for secondary epilepsy.</div>
<div id="_mcePaste">How long does epilepsy need to be treated?</div>
<div id="_mcePaste">This obviously depends on the severity and type of the epilepsy. However, most physicians would feel that a patient should be seizure-free for two to four years before stopping treatment. Stopping treatment should be a gradual process over one to three months. Never stop medication suddenly as this can lead to withdrawal fits and status epilepticus. There is no single test which will guarantee that the fits will not return.</div>
<div id="_mcePaste">What are the chances of the seizures returning?</div>
<div id="_mcePaste">This is always difficult to predict for an individual. However, over the past ten years there have been a number of long-term follow-up studies of 15-20 years duration of people with epilepsy, which have provided some answers. A good example of these studies is that by Thurston in 1982. This study was of 148 epileptic children whose anticonvulsant treatment was stopped after they had been seizure-free for four years. The children were followed up for 18 years, during which time the overall relapse rate was 28 per cent. It was also noted that:</div>
<div id="_mcePaste">two-thirds of the relapses occurred in the first two years off treatment;</div>
<div id="_mcePaste">85% of the relapses occurred in the first five years off treatment;</div>
<div id="_mcePaste">the EEG was of little use in predicting those patients who would subsequently relapse;</div>
<div id="_mcePaste">certain factors were commonly associated with relapse:</div>
<div id="_mcePaste">focal fits</div>
<div id="_mcePaste">different seizure types in one patient</div>
<div id="_mcePaste">uncontrolled fits for six years or more</div>
<div id="_mcePaste">neurological   abnormalities   on   physical   examination, suggestive of brain damage (secondary epilepsy).</div>
<div id="_mcePaste">The results of this, and other studies, suggest that people with the problems listed in the last point above are less likely to come off treatment successfully than people without such problems.</div>
<div id="_mcePaste">The outlook for different types of epilepsies</div>
<div id="_mcePaste">Grand mal seizures. In general, grand mal epilepsy is the most likely to remit (go away) and the least likely to relapse. However, this does not apply if the attacks are due to some form of structural brain damage such as after a severe head injury, damage related to birth and so on.</div>
<div id="_mcePaste">Absences (petit mal). There is some disagreement about the course which absences may take. Some believe that it occurs in children, rarely in adolescence and never in adults. Others believe it does occur in adults. The reason for this difference of opinion probably revolves around the fact that the term &#8216;petit mal&#8217; is used loosely to describe all absences, some of which are really of temporal lobe origin. It can be expected that 75% of children with absences (true petit mal) will remit completely. Children who develop absences before the age of five or after ten years of age more often have other associated seizure types (grand mal or temporal lobe) and do not respond as well to treatment.</div>
<div id="_mcePaste">Temporal lobe seizures. Temporal lobe epilepsy is frequently the most difficult type of epilepsy to treat and usually requires treatment for many years. The outlook for &#8216;cure&#8217; is slight.</div>
<div id="_mcePaste">Infantile spasms. This is a rare condition and the outlook is not encouraging. In rough terms, it can be proposed that about one-third of children will recover fully and attend normal schools, one-third will be retarded and the remaining third may be severely retarded with abnormalities of the nervous system. A proportion of children in the last two groups will develop other seizure types.</div>
<div id="_mcePaste">Lennox-Gastaut Syndrome. This form of epilepsy begins in the preschool years with several seizure types. These may include absences, drop attacks, head nodding, tonic seizures and grand mal fits. The majority of children with this syndrome are mentally retarded. Control of seizures is often very difficult and the overall outlook is not encouraging.</div>

			<content:encoded><![CDATA[<div id="_mcePaste">A long list of dos and don&#8217;ts for people with epilepsy is of little help. Most of the problems can be solved by using a bit of common sense. It is important to bear in mind that some people have mild epilepsy and others severe epilepsy. Obviously the person who has a fit once a year will approach life a little differently to someone who has a seizure once a week. The factors that need to be taken into account are:</div>
<div id="_mcePaste">the type of fit;</div>
<div id="_mcePaste">the severity of the fit;</div>
<div id="_mcePaste">when the fits occur;</div>
<div id="_mcePaste">the age of the patient.</div>
<div id="_mcePaste">When epilepsy is diagnosed, people are often shocked and frightened. This may lead to some degree of overprotection, particularly of children. But once the seizures are controlled and appropriate explanation and advice has been given, their confidence should grow and they should be encouraged to lead as normal a life as is possible. There are some practical points worthy of mention:</div>
<div id="_mcePaste">Swimming: A person with epilepsy should never swim alone. Always inform a companion of the condition and explain what to do if a seizure should occur. Avoid both scuba and springboard diving.</div>
<div id="_mcePaste">Bathing: A number of people with epilepsy have drowned in baths. Never leave them alone in the house when they are taking a bath; keep the bathroom door ajar and make sure that the bathwater is reasonably shallow.</div>
<div id="_mcePaste">Showering: The risks of showering are threefold:</div>
<div id="_mcePaste">If someone has a grand mal fit in the shower it may be difficult to get at them.</div>
<div id="_mcePaste">They might push an arm or a leg through a glass panel. Showers should be fitted with the best shatterproof glass. Wire-reinforced glass is in fact weaker than sheet glass.</div>
<div id="_mcePaste">The hot tap may be turned on fully when bumped during a fit, resulting in burns. Ideally, a temperature control device should be fitted to the water system in the shower.</div>
<div id="_mcePaste">Bicycle riding: A person with epilepsy can ride a bicycle taking the normal precautions that any other cyclist should be taking. Children who have frequent seizures should wear a helmet and ride in a protected environment.</div>
<div id="_mcePaste">Horse riding: a person with epilepsy who wishes to ride a horse should wear a helmet and ride with others.</div>
<div id="_mcePaste">Climbing: Climbing is not a sensible hobby for people with epilepsy for obvious reasons.</div>
<div id="_mcePaste">Machinery: working with power saws, presses, etc. should be avoided.</div>
<div id="_mcePaste">Employment may present problems for people with epilepsy. Obviously some occupations are just not suitable for people with epilepsy; driving a bus, working with heavy or dangerous machinery or working on a scaffold would not be sensible. Two factors need to be taken into account: the possibility that those with epilepsy may injure themselves during a seizure or that they may cause harm to others.</div>
<div id="_mcePaste">What occupations are closed to epileptics? The armed forces will not employ someone with epilepsy. Other examples include occupations such as an airline pilot, public transport driver, crane driver, etc. It is logical and appropriate that people with epilepsy should not be involved in these occupations.</div>
<div id="_mcePaste">More of a problem is the prejudice against persons with epilepsy. This leads to the perennial problem of whether people should declare their epilepsy or try to hide it. Ideally it is best to declare your epilepsy and hope that the prospective employer will understand. Sadly this is not always the case. This is a real problem and is likely to persist. It is hoped that through the efforts of National Epilepsy Associations and the various State organisations throughout the world, this problem will lessen with the passage of time as public awareness increases. Unfortunately there is no easy solution to this very real problem.</div>
<div id="_mcePaste">A further problem is that of driving. Today driving a car is an integral part of everyday living. Not being able to drive can be inconvenient and, of course, can limit job prospects. The present legal situation, which is under review, is that a person should have been seizure-free for two years to obtain a driving licence. In some situations, if seizures have habitually occurred only at night for a period of two years, the person may be granted a restricted daytime licence. There are variations from country to country and within countries amongst states and provinces. The general trend worldwide is to individualise the granting of driving licences in a commonsense way. It is important, however, to remember that driving a car is in itself a responsibility and that restrictions are for the protection of epileptics and other road users. His eyes may move rapidly from side to side (nystagmus) while trying to focus on something. If he does not react to a bright light being turned on in the room, this may indicate serious visual impairment.<br />

<p><span style="font-family:Courier New; font-size:10pt">  Here are some answers to these misassumptions regarding sexuality. In each of the twelve areas, I have included a quote from sessions with the children of the thousand couples to illustrate their concerns. The parents in the couples program were present for family sexual counseling sessions as a part of their super marital | sex program. Such sessions were optional, but 63 percent of the couples elected to have them. Fifty-two percent of those couples I requested additional sexual counseling sessions with their children, j I have found that dealing with the entire family system, not just the marital couple, is most effective in clinical work. The couples I am now seeing are electing this approach at an almost 75 percent frequency.<br />

<p><span style="font-family:Courier New; font-size:10pt">If you get turned on, you know, you get homy-like, you really don&#8217;t know what you&#8217;re doing. You might do anything. You can&#8217;t control it, so you shouldn&#8217;t get a boy turned on. You could end up attacked and not even know what happened to you.<br />

There are certain ways you have to act to belong to the right group, to fit in. We all do that, even as adults. Some girls pretend they don&#8217;t ever think about sex, because that&#8217;s what other giris say, but everyone has strong sexual feelings sometimes. There is no such thing as blue balls, and boys and girls cannot be hurt by not having intercourse after getting very aroused. As a matter of fact, feeling blue is more related to intercourse before you are really ready for it, so fooling around is a great choice if you feel enough love and respect to trust each other and really share the experience. Remember, there are ways to relieve the buildup of sexual feelings other than intercourse. Discuss some of these options with your parents.<br />

However, we should be careful which cleaning agents (solvents !) we use, because the skin also has the ability to absorb substances into our body. Techniques to accelerate skin excretion (sweating) are for example the sauna and moderate exercise<br />

Medicinal herbal cures and treatments are as old as the first amateur botanist who most likely discovered the effects of plants quite by accident. A respectable percentage of modem drugs are made from plants, or were made from plants, before they were artificially reproduced. Penicillin and belladonna are two such drugs; Valium, the tranquilizer, may have had its roots in valerian, an herb known for its identical effect. How safe, then, arc herbs for the woman with endometriosis?<br />
</span></p>
<p><span style="font-family:Courier New; font-size:10pt">Abigail Rist, a registered nurse and registered acupuncturist with a specialty of herbalism, who has worked with midwives and treated many women with menstrual disorders, believes that sufferers of endometriosis may gain some benefits from a number of herbal remedies. She follows the Chinese school of thought, viewing disease as imbalances of female (yin) and male (yang) energies, heat and cold, expansion and contraction. The herbal teas or tinctures she recommends arc based on these evaluations.<br />

<p><span style="font-family:Courier New; font-size:10pt">Since this system of medicine does not specify the disease precisely, &#8220;endometriosis is diagnosed and treated by its more irritating symptoms,&#8221; Ms. Rist told me. &#8220;In this sense, endometriosis is a tightening and over cooling of what is called &#8216;the lower burner,&#8217; or urogenital area. For this area, we prepare an herbal remedy to replenish heat (or yang) and reestablish a balance, in another example, it&#8217;s as if you have a fever or sore throat, accompanied by thirst. The remedy prescribed will bring down body heat, soothe the rawness of the throat, and help quench thirst. Dandelion tea, for example, induces these qualities. The same will be done for endometriosis.&#8221;<br />
</span></p>
<p><span style="font-family:Courier New; font-size:10pt">Using this reasoning, the overcooling effects of endometriosis would need to be treated or counteracted with warmth. <a href="http://leadmedic.com/product_info.php?cPath=60&amp;products_id=3326" title="order clomid">The herbs used would tonify the blood, stimulate and warm the organs.</a> &#8220;In traditional Chinese medicine,&#8221; Ms. Rise continued, &#8220;doctors attribute the function and regulation of some aspects of the blood to the liver and spleen. If a woman has dark clots during menstruation, it is attributed to an imbalance of the liver. If a woman has lighter red blood but profuse bleeding, this would be related to an imbalance of the spleen. The herbs prescribed would be &#8220;tissue specific,&#8217; that is. herbs with some affinity to each organ which can, if possible, influence and regulate its function.&#8221;<br />

<p><span style="font-family:Courier New; font-size:10pt">There are teas and tinctures that Ms. Rist recommends. The white peony has specific liver action, so it will actually decrease abdominal tightness and pain. The peony, which may be either dry-roasted or cured, depending on what the herbalist decides for each case, is often one of four other herbs blended in a concoction and prepared to be taken as tea. &#8220;Its nature is to be cooling,&#8221; says Ms. Rist, &#8220;so when it enters the spleen and liver channels, it can tonify the blood. The herbs can also help stop pain by relaxing the tendons and tissues.&#8221;<br />

<p><span style="font-family:Courier New; font-size:10pt">Another popularly used herb is dong quai, considered a &#8220;female&#8221; root plant, the way that ginseng it assigned male properties. Dong quai is found in many &#8220;female trouble&#8221; herb blends that are commercially made and distributed to hearth food stores. It is a cured root that, says Ms. Rist, &#8220;is thought to help establish an endocrine balance, increasing tissue sensitivity to estrogen and helps with ending menstrual cramps and pelvic congestion.&#8221;<br />

<p><span style="font-family:Courier New; font-size:10pt">Another tea suggested by this herbalist is raspberry tea, handed down as a remedy from grandmother&#8217;s rime to its use by present-day midwives. It is reputed to help relax muscles and dilate the cervix. Other teas that are toning or can help reduce inflammation or relieve pain arc rosemary tea, licorice tea, chamomile, and witch hazel tea.<br />

These are fairly common in the community, and for this reason will be mentioned. Most of the more major disorders are fortunately less common, so only some of these will be discussed.<br />

<p><span style="font-family:Courier New; font-size:10pt">Fungal infections of the nail are also a reasonably common problem. Several fungi may be involved, including the common tinea fungus responsible for &#8216;athletes foot&#8217;. Initially the infected nail is merely discoloured at the end, but as the infection spreads, it may become softened, fragile, and later thickened.<br />

<p><span style="font-family:Courier New; font-size:10pt">Eventually the nails will separate and lift off, or become grossly distorted. Most commonly, the toenails are the ones affected.<br />

<p><span style="font-family:Courier New; font-size:10pt">Unfortunately surface applications are rarely sufficient to remedy the situation, nor is removing the affected nail. The most effective treatment is regular clipping of the nail, plus the taking of specific anti-fungal tablets. Because of a toenail&#8217;s slow rate of growth, and the necessity to take the medication for the entire growth period of the nail, this treatment may be required for anything between 6-18 months.<br />

<p><a href="http://www.d-store.net/?product=eurax" title="Eurax is used for treating scabies and relieving itching."><span style="font-family:Courier New; font-size:10pt">An ingrown toenail forms when the edge of the nail penetrates the adjoining soft tissue.</span></a><span style="font-family:Courier New; font-size:10pt"> The first symptoms are redness and pain, followed by swelling and a discharge. Secondary infection then frequently occurs. The most common causes of this problem are ill-fitting shoes, nails that have been cut too short, excessive curvature of the nail, and flat feet. The first three of these problems may be avoided by wearing adequately wide and long shoes, as well as by cutting the nails straight across and not too short. Mild cases may be treated with wet dressings and packing sterile cotton wool under the affected edge of the nail. Antibiotics may also be necessary. Severe cases may require an operation.<br />

<p><span style="font-family:Courier New; font-size:10pt">A corn is a cone-shaped overgrowth of hard skin with a central core. Corns usually occur over bony prominences, such as toe joints, or between the toes. They are a result of chronic pressure or rubbing for long periods. Poorly fitting shoes are the most common cause, but arthritis, improperly positioned toes or bony spurs may also be involved. Prevention requires shoes to fit properly and, occasionally, mechanical devices to position the toes correctly. Otherwise the corn may be padded over or gradually pared down. The latter may be done with either a salicylic acid paste or the help of a podiatrist. If there is an underlying bony spur, then an orthopaedic surgeon may have to deal with it.<br />

<p><span style="font-family:Courier New; font-size:10pt">Psoriasis frequently affects the nails, resulting in deformities which may precede all other manifestations of the disease. Although the nails may occasionally be the only part of the body affected, in most cases of psoriasis, the symptoms are much more widespread. Almost all patients with psoriasis find their nails are affected at some time during the course of their disease. This, however, may be quite transient. The character istic changes are fine thimble-like pitting, lifting, discolouration, and gross thickening of the nails. Treatment of nail psoriasis is most unsatisfactory. Some cases clear spontaneously, whereas others respond to rather painful cortisone injections into the nail-fold.<br />

<p><span style="font-family:Courier New; font-size:10pt">Eczema or dermatitis affecting the hands will often affect the nail as well. The usual symptoms are transverse ridging, coarse pitting, discolouration, and eventually marked deformity of the nail. This will all correct itself once the eczema or dermatitis is controlled.<br />
